The Oak Dance liability release form needs to be signed by every dance participant. Note that you must fill out the liability form for your specific dance location. The correct form for you will come attached to your completed registration email.

For Longbranch participants, the forms will taken care of at the dance.

 

For Northern California participants returning your completed Liability Release Form can be done in one of two ways.

1. Print out the form, sign it, and either scan it and email it to north@oakdance.org  or send by regular mail to:
